Online Patient Forms

Your First Visit:

When you arrive on the day of your first appointment you will check in at the desk, where one of our staff members will verify your information and collect any copays required by your insurance plan. After that one of our medical assistants will look to get a brief medical history and discuss what your dermatology needs or concerns are. Finally, you’ll get a chance to sit down with one of our providers to begin discussing how we can help you get the skincare you deserve.

It is our belief that the key to better healthcare is helping patients and providers develop good relationships, which is why we aim to make that first appointment as quick and painless as possible. When patients and providers spend more of their visits face to face, patients see better care. Call now at and start your journey to better skincare.

What To Expect

Being well-prepared for your appointment will ensure that the provider has all of the needed information to provide the best possible care for you. If you are having a skin exam please do not wear make-up on the area(s) to be examined. You can educate yourself on your symptoms by reviewing the content on this Web site. We look forward to your first visit.

Please remember that we have multiple providers at all our locations.  You may not be called back to an exam room in the order you checked in.

Appointments

We know you have many choices when choosing a dermatologist in Winston-Salem, NC so we have made  requesting an appointment  a simple and convenient process via our Web site. Please be aware that some insurance plans require a referral from your Primary Care Provider (PCP). We will need the referral from your PCP prior to scheduling your appointment. Appointment Reminders

We will call and text you reminders for your appointment. If you want to opt out of the reminder calls, please inform our check-in staff when you are here for your appointment. Remember, we require a minimum of 24 hours advance notice of cancellation to avoid a no-show fee.

Insurance and Billing

We accept most traditional insurance plans and will bill your insurance carrier(s) on your behalf. If you are unsure whether or not we are in network with your plan, please contact your insurance company so they can give you information specific to your plan. Please note that many insurances cover our practice as a whole, rather than each individual provider by name. Procedures may be processed differently than office visits by your insurance company; this is dependent upon your individual plan.
